I fully understand, acknowledge and agree that I am undertaking training to expand my knowledge and skill set relative to PEMF. The information provided and techniques that I am learning do not qualify me to perform or provide any type of veterinary medical practices or procedures. I am not qualified to diagnose any medical condition or to recommend any veterinary medical procedures.
I am of lawful age and in consideration of being permitted to participate in this program, I release and forever discharge the training provider, their spouse, heirs, executors, administrators, legal representatives, and assigns from all manner of actions, causes of action, debts, accounts, bonds, contracts, claims and demands for or by reason of any injury to person, animal or property, including injury resulting in the death of the participant or any animal, which has been or may be sustained as a consequence of the participants participation in this training program and not withstanding that such damage, loss or injury may have been caused solely or partly by the negligence of the training provider.
I release and discharge and to hold harmless and indemnify Debra Pearse (o/a Pawsitive Spirit and AnimalPEMFTherapy.com), it’s employees, volunteers, and agents, and the owners of the premises or grounds upon which the event or training session is held, including that of on-line digital classrooms and their respective employees and agents, collectively called the “Releasees”, of and from any and all claims, and from all liability of any nature, for all Loss which I or any animal that I work with under my control, including specifically, but without limitation, all Loss resulting from the action or inaction of any animal, or by the Releasees, or any of them, whether through negligence or otherwise. I expressly assume the risk and liability for all such Loss while attending or in proximity to this event or training session – on-line or in person - and waive any and all claims relating thereto;
The participant acknowledges to the training provider that the participant does not have any physical limitations, medical ailments, or physical or mental disabilities that would limit or prevent the participant from participating in this training program.
That all indemnities herein shall be full and complete indemnities, including legal costs on a solicitor and client basis.

I have been given the opportunity to read this form before signing it, that I have in fact done so, that I fully understand the contents of this form and that it restricts my legal rights, and that I am signing this form voluntarily and of my own free will.

I fully understand the risks and claims that I am waiving to participate in this training program.
I am freely and voluntarily executing this waiver.
I am forever prevented from suing or otherwise claiming against the training provider as outlined in this document for any property loss or personal injury that I may sustain while participating in or preparing for this training program.
